Revised R104 Renewal Application Form for Fellow Membership for your attention. 

Kindly please be reminded to submit CNE records as well.

There is a non-renewal section included on this revised year 2020 form, please fill out the form accordingly.

Deadline for return of renewal application: on or before 31 March 2020

If you are our Retired Fellow Member, please submit the captioned forms. Note that the renewal fees is: $1000 for Retired  member.

Please note that an administrative charge of $200 member will be imposed to the membership renewal received after 30 April 2020.
